Astoria Nursing and Rehabilitation is a 218 bed skilled nursing facility serving the San Fernando and Santa Clarita Valleys, conveniently located within close
proximity to major medical centers. Astoria maintains the highest level of ethical standards, clinical expertise and professional services and strives to meet the needs of the long term resident requiring 24-hour monitoring and assistance with various activities of daily living to the short-term medically complex
patient requiring rehabilitation or post-surgical recovery and treatments. We offer private and semi-private accommodations and are contracted with numerous HMO’s and medical groups. Astoria has registered and licensed vocational nurses on staff 24 hours a day, 7 days a week. Certified nurses’aids are also available to meet the additional needs of our residents and patients.
Upon admission, a team of professionals develop a medical assessment and a care plan tailored to the resident’s needs. Team members coordinate therapies, social activities and diets for each resident to ensure a full and comfortable lifestyle as well as meeting the physical, emotional and social needs of the resident.

We have created a homelike environment where residents are treated in comfort and security. An emergency call system links each bed with the nursing station, which is staffed 24 hours per day for immediate response to each resident’s call.
